FSPH Pre-major Request Form

The Fielding School of Public Health (FSPH) is now accepting requests from first-year students (those admitted in Fall 2022 from high school) who wish to declare the public health pre-major. Complete this form for consideration. Other students can contact undergrad_admissions@ph.ucla.edu to discuss eligibility for the majors.
